Life Insurance
Grace offers several options so you can choose the level of coverage that best meets your family’s needs.
- Basic Life Insurance: Coverage provided by the company at no cost to you
- Benefit amount is 2x’s salary up to $100,000
- Basic Life benefit is paid to your beneficiary if your death is the result of illness or natural causes
- Supplemental Life Insurance: Optional coverage you can purchase for additional protection
- Dependent Life Insurance: Coverage you can elect for your spouse and/or eligible children
- Accidental Death & Dismemberment (AD&D): Extra protection in case of accidental death or serious injury paid to your beneficiary
Voluntary Life & AD&D Insurance
In addition to the company-paid Basic Life coverage, you have the option to purchase extra protection for yourself and your family. This added coverage gives you the flexibility to choose the level of financial security that best fits your family’s needs.

Voluntary Disability Insurance
Disability insurance helps replace part of your income if you are unable to work due to illness, injury, or accident. This coverage is employee-paid, giving you the option to add financial protection that fits your needs.
- Short-Term Disability (STD): Provides income replacement for a limited time (up to 11 weeks) while you recover
- Long-Term Disability (LTD): Offers extended income replacement if you are unable to return to work for a longer period. Benefits start after 90 days.
By enrolling in disability coverage, you can help ensure that you and your family have continued financial support during unexpected health challenges.

Starting a Disability Claim
If you need to file a Short-Term or Long-Term Disability claim, the process is simple:
- Complete the Disability Claim Form and submit it to Mutual of Omaha.
- Mutual of Omaha will be in touch with you to guide you on next steps.
- Contact HR to let us know about your leave and coordinate available pay (PTO, maternity pay, etc.) with your disability benefit.
Starting your claim as soon as possible helps avoid delays in receiving benefits.
